Affiliates in Plastic Surgery, LLC (“the Practice,” “we,” “us,” or “our”) values your privacy. It does not apply to information collected offline, information collected in person at any of the Practice’s locations, information collected through via telephone, information collected through patient intake or insurance forms that you might find on the website but submit outside of the website, or information collected through third-party platforms, including the patient portal used by the Practice, which may be governed by its own privacy policy.

 

2. Information We Collect.  We may collect the following types of information from visitors to our Site:

 

a. Information You Provide Directly.  If you fill out a contact form, we may collect your name, email address, phone number, and any other information you choose to provide in the comments section. That said, we request that you do not submit sensitive personal, medical, or financial information through the Site.
